Offering care for chronic conditions through telemedicine is an innovative way to treat people where they are. Resolute Nursing Solutions launched in response to a need we saw within our own community. Resolute Nursing Solutions provides healthcare through telemedicine, with a delivery model specifically designed to address health disparities in the Black community. Telemedicine, provided through HIPAA-compliant platforms, allows us to reach people who would not otherwise have reliable access to health care, due to financial, physical, geographic, or cultural barriers.
Resolute Nursing Solutions is experienced in treating patients of all ages, from children to older adults. We guide the care of individuals with chronic and/or complex health conditions, such as asthma, diabetes, and high blood pressure, which data show are prevalent among many individuals lacking access to traditional in-person health care. As a point of contact for medication refills and nursing care, Resolute Nursing Solutions will provide case management to residents in a geographic region lacking access to adequate health care.
